Accepting fully potty trained 4 and 3-year-old children (must turn 3 by Sept.1, 202___).
Please bring the following documents:
- Child's birth certificate
- Siblings birth certificates
- Proof of income for the family
- Immunization record (yellow card)
- Address verification
- Form CD 9600 (Provided at the Child Development Office)
Completing the CD 9600 application does not guarantee enrollment into the State Preschool Program. You will be notified within 30 days of completing the CD 9600 of your eligibility status.
